Capture leads from your website with CRM contact forms

Embed a Hive CRM contact form on your website so that each submission creates or updates a person in CRM, with an owner and a lead source.

Written by Ellen Kajca

Use CRM contact forms to capture leads from your own website. You create the form in Hive, copy a short code snippet, and paste it into your site. When a visitor sends the form, Hive adds them to People in CRM and emails the right owner.

Not the same as Hive Forms. CRM contact forms are embedded on your website and create CRM people. To build request forms that create actions or projects, see Hive Forms.

Before you start: Your workspace must have CRM turned on. Any member with CRM access can create and manage contact forms; you don't need to be an admin.

Create a contact form

  1. Open Workspace settings → Apps.

  2. Scroll to the CRM section and find the Contact forms card.

  3. Click New form.

  4. In the New contact form window, fill in the settings:

    • Name — the internal name of the form. This is required.

    • Enabled — leave on. Disabled forms reject submissions.

    • Title and Description — the text at the top of the form.

    • Submit button label — the text on the button.

    • Success message — the text that the visitor sees after they send the form.

    • Scheduling link (Calendly) — optional. If you add an https link, the visitor sees your Calendly page instead of the success message, with their name and email filled in.

    • Default owner — the CRM user who gets new leads from this form. Leave it as Workspace default to let Hive choose.

    • Allowed domains — the websites that can send this form, such as example.com, one per line or separated by commas. Subdomains such as www.example.com are included. Leave empty to allow any domain.

  5. Optional: under Form fields, click Add custom field to add more questions. See Add your own fields below.

  6. Click Create form.

The form shows in the Contact forms list with an Active or Disabled badge.

Add your own fields

Every form has these fields. You can't remove or rename them.

  • Name (required)

  • Email (required)

  • Company

  • Phone

  • Message (always last)

To ask more questions, add a People custom field:

  1. In the form window, click Add custom field.

  2. Enter a field label.

  3. Under People custom field, select the field. You can use Text, Number, Date, and Select fields. A Select field accepts one choice.

  4. Optional: add a Placeholder and turn on Required.

If the list is empty, create the field first in Workspace settings → Custom fields, on the People tab.

Add the form to your website

  1. In the Contact forms list, click Embed code next to your form.

  2. Click Copy code.

  3. Paste the snippet into your website page where the form should appear.

  4. Publish the page and send a test submission.

If you set Allowed domains, make sure that your website's domain is in the list. Otherwise, the form rejects the submission.

Track where leads come from

The Embed code window also shows a second snippet under Track lead source on every page.

  1. Click Copy next to Track lead source on every page.

  2. Add this snippet to every page of your website (for example, in your site's footer or custom code settings).

This snippet remembers, in the visitor's browser, which page or campaign brought them to your site. Check whether your site needs the visitor's consent before you add it. The snippet waits for consent tools such as Cookiebot or OneTrust when they hold it back.

Without this snippet, Hive can only see the visit to the page with the form. If the visitor reached the form from another page of your site, Hive records no lead source.

What happens when someone sends the form

A new contact

If the email address is not in your CRM yet, Hive:

  • Creates a person in People with their name, email, company, and phone.

  • Saves their message in the person's Notes.

  • Saves your custom field answers on the person.

  • Sets Created via to Form.

  • Sets the Contact owner to the form's Default owner. If the form has no default owner, or that person no longer has CRM access, Hive uses the workspace default: the first person on your workspace's CRM access list, or a workspace admin.

  • Links the person to an existing account when the Company they entered matches an account name. Capitalization is ignored.

  • Emails the contact owner.

A contact who is already in CRM

Hive finds the person by email address. Hive does not create a duplicate. Hive:

  • Fills in a blank company, notes, or custom field. Hive does not change data that your team already has.

  • Adds a new phone number to the person if it is different from the ones they already have.

  • Links an account only if the person has none yet.

  • Keeps the person's Contact owner.

  • Adds an entry to the person's History tab: "Hive recorded a website form submission", with the form name and the page.

  • Emails the person's contact owner and the form's default owner. If they are the same person, they get one email.

  • If the person has no contact owner, emails the form's default owner (or the workspace default, if the form has none).

The notification email

The subject is CRM form submission: followed by the form title. The email shows the visitor's name, email, company, phone, message, and the page they sent the form from. Custom field answers are not in the email; open the person in CRM to see them. Click View in Hive CRM to open People.

See the lead source

Open the person in People. When Hive has a lead source, the Lead source section shows two columns:

  • Original — how the person first found your site. This does not change after Hive records it.

  • Latest — the visit that led to their most recent form submission. This updates when Hive can see where the visit came from.

Each column shows Channel, Source, Campaign, Landing page, Form page, and Date.

The Channel is one of these values: Paid search, Organic search, AI assistant, Paid social, Social, Email, Referral, Direct, or Other.

Source is the campaign's utm_source value or, if there is none, the website that referred the visitor.

Filter People by lead source

In People, add the Original channel and Original source columns. These columns are hidden by default. You can filter the list by Original channel, for example to show all Paid search leads.

Start a workflow from a form submission

To run a workflow each time someone sends a contact form (for example, to enroll them in a sequence):

  1. Create a workflow and choose the Contact form submitted trigger under System events.

  2. Under Contact forms, select one or more forms. Leave empty to run for any contact form.

This trigger runs for new and existing contacts. It must be the only trigger in the workflow.

Edit, turn off, or delete a form

  • To edit a form, click the pencil icon next to it, make changes, and click Save changes. You do not need to update the code on your website.

  • To stop a form temporarily, edit it and turn off Enabled. Disabled forms reject submissions.

  • To delete a form, click the trash icon and confirm. The form stops accepting submissions and its embed code no longer works.

Good to know

  • Name and Email are always required. A submission without an email is rejected.

  • A repeat submission never overwrites existing person data.

  • The lead source is recorded only for submissions through the website form, not for people you add manually or import.

  • Changes to a form's settings take effect the next time someone loads the page, because the snippet loads the current form from Hive.

  • If a select option or custom field changes after you build the form, Hive skips answers that no longer match the field.

  • Forms have no CAPTCHA or consent checkbox. Use Allowed domains to limit where the form can be sent from, and add any consent text your site needs to the form's Description.

  • To limit spam, a visitor can send up to 10 submissions a minute, and each form accepts up to 60 a minute.

Troubleshooting

"This site is not authorized to submit to this CRM form"
The website's domain is not in the form's Allowed domains. Add it, or leave the list empty.

"Form is not available."
The form is turned off or deleted, or CRM is turned off for the workspace.

"CRM form requires a valid email address"
The visitor left the email blank or entered an address that isn't valid.

"Too many CRM form submissions. Please try again later."
The visitor or form reached the submission limit. Wait a minute and try again.

A lead has no lead source.
Add the Track lead source on every page snippet to every page of your site. Without it, a visitor who moves between pages of your site before sending the form has no lead source.
