Add questions in the Form builder tab of the form editor. To change the options of a question, select the question and click Form settings. The options depend on the question type. For example, you can make a question required, add help text, or set a maximum number of characters.
For how to create and publish a form, see Hive Forms.
Question types
Click or drag a question type from the left side of the form editor to add it to the form. You can add:
Answer fields: single-line input, text area, multi-text field, radio button group, checkboxes, dropdown, multi-select dropdown, date, and file upload
Layout items: ruler (divider), header, paragraph of text, image, and panels
Dynamic fields: users, teams, projects, and more. See Dynamic fields in Hive Forms.
These examples show some of the question types:
Duplicate a question
Select a question in the Form builder and click Duplicate at the bottom of the question card. The copy keeps the logic of the original question.
Date questions
Limit which dates people can select
Select the date question and click Form settings. Under Set Dynamic Date Range, enter a Min date, a Max date, or both. Enter each value as a number of days from the submission date.
Allow or block weekends and holidays
Select the date question and click Form settings.
In the Data section, select Include holidays, Include weekends, or both. Clear them to block those dates.
If the form creates actions, also check the weekend setting where Hive creates them:
Action in a specific project: open the three dots menu of the project, select Edit project > Actions > Action card settings, and turn on Allow actions to be assigned on weekends.
Action without a project, or a new project: go to Workspace settings > Defaults > Project default settings and select Allow actions to be assigned on weekends. If the form creates the project from a project template, change this setting in the project template settings instead.
To create an action only for some dates, see Create an action based on a date.
Limit the file types a file upload accepts
By default, a file upload question accepts all file types. To accept only some file types:
Select the question in the Form builder and click Form settings.
In Accepted file categories, select one or more categories: Images, Videos, Audio, Documents, or Archives.
To accept other file types, also select Custom. Then enter the file types in Additional file extensions, which shows after you select Custom. Enter file extensions that start with a dot, such as .docx or .indd, or file types such as application/pdf or image/*. Put a comma between values.
Save your changes, then click Publish or Save and Publish.
Do not enter names such as "Word Document" or "PDF". To accept Word files, enter .doc, .docx. If a value is not valid, Hive shows an "isn't a file type" error. You cannot publish the form until you correct or clear the value.
Which files are accepted:
A file is accepted when it matches a selected category or a value in Additional file extensions.
If you select no category and Additional file extensions is empty, the question accepts all file types.
If Map to action proofs is on and Proofing is on in your workspace, the question accepts only file types that Hive proofing supports. If you also selected categories or extensions, it accepts only the selected types that proofing supports. If proofing supports none of them (for example, you selected only Audio), the question accepts no files.
Fix a form that rejects every file
A form that you published before October 29, 2026 can still have a value that is not valid. On that form, submitters see "This file type is not allowed. Choose a supported file." for every file they upload. To fix it:
Open the form in the Form builder.
Select the file upload question and click Form settings.
In Additional file extensions, replace the value with file extensions, such as .doc, .docx. Or clear the value to accept all file types.
Click Save and Publish.
Open the published form and upload a test file.
Choose who appears in a user field
Use a Dynamic users question when respondents must select a person, such as the person who requests the work or the assignee of the action. In the form editor, select the question and look at Available users.
For a question that is not mapped to Action assignee:
Workspace members: includes internal workspace members, even if they are not members of the destination project. Does not include external users.
All: includes internal workspace members and external users. When the form creates an action in an existing project, only external users with access to that project are included.
Fields mapped to Action assignee
This rule applies when a user question is mapped to Action assignee and the form creates an action in an existing project. Then Workspace members and All include only people with access to that project. All does not let you assign people who do not have project access.
For example, a Requested by question can list internal members from all of your workspace. A question mapped to Action assignee lists only people with access to the destination project.
Which project does Hive use? The user list uses the existing project in the first create-action target in the submission settings. Do not expect the list to change to match a project that the respondent selects in a different question.
Why is someone missing from the dropdown?
Check the Available users setting of the question. Workspace members does not include external users.
Check if the question is mapped to Action assignee.
Check the destination project in the first create-action target in the submission settings.
For an external user, or for a question mapped to Action assignee, make sure that the person has access to that project.
After you change the form, save and publish it. Then test the dropdown in the published form.
Format text in answers
People who fill out a form can make text bold, italic, or underlined in text area, single-line input, and multi-text field answers. They select the text and press Ctrl+B, Ctrl+I, or Ctrl+U (Command on Mac). The formatting stays in the action or project that the submission creates.
